What you will need to enroll:
Proof of San Mateo residency:
- Pay stub that includes your home address, or
- Utility bills, or
- Rental agreements, or
- Driver's license or state identification card, or
- Voided personal check with address
Proof of income:
- Copy of a recent pay stub, or
- Employer statement of gross monthly income that includes the time period covered, or
- Last year’s federal income tax return, or
- For those without written proof of income, a letter from a charitable organization, or
- Self-employed persons can include last year’s federal income tax return including Schedule C or the last three month’s net profit and loss statement.
Other information:
- Proof of your child's age such as a birth or baptism certificate
- Your child’s Social Security Card number (optional)
