Do I Qualify?

Individuals who do not have health insurance through an employer or through an individual insurance plan, may be eligible to receive health benefits through a program offered by the Health Plan of San Mateo.

Eligibility criteria for most HPSM programs is determined by state and locally funded health insurance and coverage programs. Residency in San Mateo County is required for most HPSM programs.

CareAdvantage combines your Medicare and Medi-Cal benefits in one simple plan. This plan offers lots of additional benefits, like free taxi rides to approved medical appointments, vision care, and coverage for different types of long-term care services.

To qualify for CareAdvantage you need to have Medicare Parts A & B and full-scope Medi-Cal. You must also be a resident of San Mateo County. People with developmental disabilities are eligible to apply.

Do I Qualify for CareAdvantage (HMO SNP)?

CareAdvantage (HMO SNP) is a Coordinated Care plan with a Medicare contract and a contract with the California Medicaid program. Enrollment in CareAdvantage (HMO SNP) depends on contract renewal. To qualify for CareAdvantage CMC you need to have Medicare Parts A & B and full-scope Medi-Cal. You must also be a resident of San Mateo County. Persons with End-stage Renal Disease (ESRD) are eligible to apply.

CareAdvantage Cal MediConnect Plan (Medicare-Medicaid Plan) is a health plan that contracts with both Medicare and Medi-Cal to provide benefits of both programs to enrollees.
